appropriate authorizations) and submitted to AUL through regular
mail for processing. If your plan participants need assistance
completing these forms, they are welcome to contact our Participant
Service Center at 1-800-249-6269 during normal business hours (8
a.m. to 8 p.m. Eastern Time).

Distribution Assistance is a free service
provided by AUL's Participant Service Center.
This Guide should be shared with participants who are retiring or
terminating employment,
to help them utilize AUL's Distribution Assistance service.

This worksheet is for the plan administrator. Please have the
participant complete and sign this worksheet for your use when
requesting their loan in eSponsor. Participant e-mail is requested
for electronic delivery of loan paperwork to the participant. If
participant e-mail is not provided, the loan paperwork and check
will be sent to the employer.

Spousal Consent for Loan Distributions
(Please see your plan document for applicability)
Completion of this form may be required if the participant's vested
account balance is greater than $5,000. This form gives spousal
consent to the borrower's use of his or her accrued benefit in the
plan to secure the borrower's loan obligations. The plan
administrator or a notary public must witness the signature of the
participant and spouse. Unmarried participants must certify their
marital status on the spousal consent form. The plan administrator
should keep this form for his or her files and does not need to
mail a copy to AUL.
