The following forms are available for your convenience. These forms can be printed, completed (including appropriate authorizations) and submitted to AUL through regular mail for processing. If your plan participants need assistance completing these forms, they are welcome to contact our Participant Service Center at 1-800-249-6269 during normal business hours (8 a.m. to 8 p.m. Eastern Time).

Distribution Assistance Guide
Distribution Assistance Guide (Spanish)

Distribution Assistance is a free service provided by AUL's Participant Service Center.
This Guide should be shared with participants who are retiring or terminating employment,
to help them utilize AUL's Distribution Assistance service.

  • Annuity Request
    (Please see your plan document for applicability)

    This form should be used and completed by the participant and
    accompanied with the Benefit Distribution Request form for:

    • the purposes of establishing an annuity upon a participant's retirement.
  • Automatic Rollover IRA Agreement

    This form should be completed and submitted for:

    • the purposes of establishing an Automatic Rollover IRA Agreement between Plan Fiduciary and American United Life Insurance CompanyŽ (IRA Provider).
  • Benefit Distribution Request for Deferred Compensation Plans
    (Please see your plan document for applicability)

    This form should be completed and submitted for:

    • standard terminations of employment;
    • terminations meeting retirement criteria;
    • disability benefits;
    • death benefits;
    • withdrawals meeting unforeseeable emergency criteria;
    • 70.5 benefits;
    • required minimum distribution benefits.
  • Full Distribution Request
    (Please see your plan document for applicability)

    This form should be completed and submitted for:

    • standard terminations of employment;
    • terminations meeting retirement criteria;
    • disability benefits.
  • Partial Distribution Request
    (Please see your plan document for applicability)

    This form should be completed and submitted for:

    • hardship withdrawals;
    • required minimum distributions (age 70-1/2 distributions); or,
    • permitted in-service distributions after age 59-1/2.
    • death benefits;
    • involuntary cashouts; or,
    • distributions due to a Qualified Domestic Relations Order.
  • eLoan Worksheet

    This worksheet is for the plan administrator. Please have the participant complete and sign this worksheet for your use when requesting their loan in eSponsor. Participant e-mail is requested for electronic delivery of loan paperwork to the participant. If participant e-mail is not provided, the loan paperwork and check will be sent to the employer.

  • New Hires

    Add new hires with your ongoing contribution submission or using the eSponsor Web site under "Participant Data - Add New Employee" option.

  • Participant Status Sheet

    This form should be completed and signed by plan sponsors for:

    • standard terminations of employment; or
    • status changes other than terminations.
  • Spousal Consent for Loan Distributions
    (Please see your plan document for applicability)

    Completion of this form may be required if the participant's vested account balance is greater than $5,000. This form gives spousal consent to the borrower's use of his or her accrued benefit in the plan to secure the borrower's loan obligations. The plan administrator or a notary public must witness the signature of the participant and spouse. Unmarried participants must certify their marital status on the spousal consent form. The plan administrator should keep this form for his or her files and does not need to mail a copy to AUL.
